POLICY
Refuge House is dedicated to ensuring the confidentiality of those we serve. Refuge House understands that all identifying information including paperwork, photographs, and other means of information is confidential and can not be released without prior written authorization. (AS 13.06)

Adoption Worker
Upon receipt of request for information, obtain consent to release document/information in the form of a Consent for the Release of Information Form from the individual for whom the information has been requested.
The Release of Information form should indicate specific documents/information being requested. Only that specific information will be released. The signed release will be attached to the individual’s file.
If the individual declines to sign the Release of Information Form, no information will be released.
If an individual wishes to waive his/her confidentiality, an informed consent will be obtained in the form of a notarized affidavit before the release of any information is initiated. The notarized form will be attached to the individual’s file.
